1. Registration form.
    
2. Statute of the company or amendments of it, in which its partners appear [General partnership (Ο.Ε.) - Limited partnership (Ε.Ε.) / Official Government Gazette (Φ.Ε.Κ.) of its constitution and one in which its administrators appear or statement of its constitution and definition of administrators issued from the General Commercial Registry (Γ.Ε.ΜΗ.) [Limited liability company (Ε.Π.Ε.)]  / Official Government Gazette (Φ.Ε.Κ.) of the Management Board's assembly or statement of its constitution, as much as the data of the Management Board's assembly, issued from the General Commercial Registry (Γ.Ε.ΜΗ.) [Shareholding company (Α.Ε.)] / Statement of its constitution and definition of administrator issued from the General Commercial Registry (Γ.Ε.ΜΗ.) [Private company (Ι.Κ.Ε.)].
    
3. Certificate of the Tax office or other document of the Tax office (e.g. business startup), which affirms that the personal enterprise or company operates as a producer - installer - importer of scaffolding, for at least three years.
    
4. Certificate or other equivalent document from the relevant chamber (commerce, small & medium industries e.t.c.), which affirms that the personal enterprise or company operates as a producer - installer - importer of scaffolding, for at least three years.
    
5. Affirmation of the candidate member that he/she is not prosecuted, and has not been convicted of fraud, forgery and fraudulent bankruptcy offenses.

* The documents 2 are required as long as the candidate member is a partner member of a General partnership company (Ο.Ε.) or a Limited partnership company (Ε.Ε.), a partner-administrator of a Limited liability company (Ε.Π.Ε.) or a Private company (Ι.Κ.Ε.) or a member of the Management Board of a Shareholding company (Α.Ε.).

** If the candidate member has not completed three years in the profession, but continues a previous personal enterprise or company that comes from the conversion of another company or personal enterprise, the documents 3 and 4 will be submitted for this personal enterprise or company.

*** The documents 3 and 4 are submitted in each case, even if the candidate member or the candidate company has neither completed three years in the profession nor comes from the continuation or conversion of a personal enterprise or company that has completed three years in the profession.
